A normal platelet count does not mean you can safely have surgery. Platelets are only the first step in stopping bleeding. The second step — the clotting cascade — is measured by a different set of labs entirely: PT, PTT, and INR. These three numbers reveal a bleeding risk that platelet counts alone will miss.
The Clotting Cascade in One Paragraph
When a blood vessel is injured, platelets form the first plug. That plug is then reinforced by a chain reaction of clotting factors — proteins made mostly by the liver — that convert prothrombin to thrombin and, ultimately, fibrinogen to fibrin. Fibrin is the mesh that holds a stable clot together. That chain reaction has two entry points, the intrinsic pathway and the extrinsic pathway, both of which feed into a shared common pathway.
The Three Numbers
- PT (Prothrombin Time): ~11–13.5 seconds. Measures the extrinsic and common pathways.
- INR (International Normalized Ratio): ~1.0. A standardized version of PT that lets results be compared across labs. Especially important for patients on warfarin.
- PTT (Partial Thromboplastin Time): ~25–35 seconds. Measures the intrinsic and common pathways.
A prolonged number means the blood is taking longer than expected to clot in that pathway.
Where Common Medications Act
- Warfarin (Coumadin): Blocks the production of vitamin K-dependent clotting factors — II, VII, IX, and X. Monitored by PT/INR.
- Heparin (including low molecular weight heparins): Enhances antithrombin, which inhibits thrombin and factor Xa. Monitored by PTT.
- DOACs — direct oral anticoagulants — such as rivaroxaban (Xarelto) and apixaban (Eliquis): Directly inhibit factor Xa in the common pathway. Not routinely monitored by PT/PTT.
- Aspirin and NSAIDs (ibuprofen, naproxen): Do not act on the cascade directly. They block platelet aggregation via COX-1. Their effect is on primary hemostasis, which is why platelet count can look normal but bleeding is still prolonged.
Why It Matters for Cosmetic Surgery
If PT, PTT, or INR is prolonged going into surgery, the blood will take longer to clot at every point of the procedure. The result is more intraoperative bleeding, larger hematomas, higher revision rates, and — in the worst case — a return trip to the operating room.
Before elective surgery, these labs must be in range. Any medication that affects them needs a planned hold and restart schedule, coordinated between your surgical team and the prescribing provider. Do not stop a blood thinner on your own — some medications require a bridging protocol.
What Patients Should Know
- Disclose every anticoagulant, antiplatelet, and supplement. Fish oil, turmeric, garlic, ginkgo, and high-dose vitamin E can all prolong bleeding. - Confirm your hold instructions in writing. - Bring an up-to-date medication list to every pre-op visit. - Never assume a "natural" supplement is bleeding-neutral.
