If you are a patient requesting access to, or correction of, your medical records or any protected health information (PHI), please email records@elevemd.com instead. HIPAA record requests follow a different process and typically require identity verification. We respond to HIPAA record requests within 30 days as required by 45 CFR 164.524.

Your rights under the California Consumer Privacy Act
The California Consumer Privacy Act (CCPA) provides you with rights that determine how your personal information is treated. California residents can choose to opt out of the sale of their personal information to third parties. Additionally, they can request to unsubscribe from all communication and ask that their personal information be deleted.

Make a Data Request
We respond to verifiable consumer privacy requests within 45 days of receipt, as required by the California Consumer Privacy Act (Cal. Civ. Code §1798.130). If additional time is needed, we may extend this period by an additional 45 days and will notify you of the extension and the reason in writing.
