This HIPAA Privacy Policy describes how Elevé Cosmetic Surgery ("we," "us," or "our") may use and disclose your protected health information (PHI) and how you can access your information. When Authorization is Required
We will not use or disclose your PHI for the following without your explicit written authorization:

- Most disclosures of psychotherapy notes

You may revoke your authorization at any time in writing, except to the extent that we have already acted based on your authorization.

You have the right to:
- Request Restrictions on certain uses or disclosures of your PHI
- Receive Confidential Communications in a certain way or location (e.g., home vs. work)
- Inspect and Copy your health records (fees may apply)
- Request Amendments to your health information if you believe it is incomplete or incorrect
- Receive an Accounting of Disclosures we've made of your PHI in the past six years (excluding treatment, payment, and operations)
- Obtain a Paper Copy of this Privacy Policy, even if you agreed to receive it electronically
